Care Management Overview
The Agency on Aging’s care management programs help keep you at home for as long as possible. Our care managers work with you to find out what you need, and then create and implement a plan for services. We follow-up with you regularly to ensure your care plan reflects your current needs. Multipurpose Senior Services Program (MSSP)

This care management program, commonly called MSSP, provides an in-home alternative for individuals age 65 and over who are frail, certifiable for placement in a long-term care facility, and eligible for Medi-Cal.
Linkages Care Management Program
The Linkages care management program serves persons age 60 and over who are experiencing increasing difficulty with physical or cognitive functions, have health conditions, or have other complex needs that place them at high-risk for placement in a long-term care facility.
