Eligibility: Caregiver 18+ providing care to a senior 60+, caregiver 18+ providing care to individual with Alzheimer’s disease or a related disorder with neurological brain dysfunction, an older adult relative caring for an individual with a disability

Description: The primary objective of the caregiver case management program is to provide assistance either in form of access or care coordination in circumstances where caregivers are experiencing diminished functioning capacities, personal conditions or other characteristics, which required the provision of services by formal service providers.

Description: Promotes independence through arranging and coordinating services to help individuals with disabilities and the frail elderly to remain safely at home. A comprehensive care management program designed to help “fill in the gaps” by serving individuals who are not eligible for other care management programs.

Description: Provides social and health care management for frail elderly clients who are certifiable for placement in a nursing facility but who wish to remain in the community.

Fee: $250- $750 initial assessment. $50 - $200 per hour for conultation. $75 per hour for ongoing case management.

Description: RN consultation, assessment, evaluation of physical, In-Home safety evaluations and mental health needs of client. Assist with life planning issues. Creates a care plan, medication and pain management, arrange for services, and placement issues. Assists with mental health problems due to loss, depression, transitional issues and grieving. Provides screening of caregivers for clients and families who areunder OACS care management, assist with placement as needed.
