Case Management - Enables functionally-impaired
older persons to obtain services which promote and maintain
optimum level of functioning in the least restrictive
setting.

Homemaker (In-Home Services) -
Housekeeping services provided to frail seniors in order
to maintain them safely in their home environment. Includes
vacuuming, dusting, mopping, laundry, dishes, meal preparation,
yard work, and sometimes transportation and grocery
shopping.

Adult Day Care - Provides socialization
and activities for frail seniors and respite for their
caregivers. This enables them to stay in the community
rather than being placed in a long-term care facility.

Transportation - Escort transportation
from home to appointments is provided in Chowchilla
and Madera; meal site transportation is provided throughout
Fresno County and the City of Fresno.

Legal Assistance - Free assistance
in civil matters, including tenant/landlord problems;
Social Security/SSI; disability; food stamps; general
relief; Medi-Cal; unemployment insurance benefits; health
care issues; immigration; senior citizens problems;
education/youth; labor; homeless issues and temporary
restraining orders if their is physical abuse.

Ombudsman Services - Trained
and certified volunteers monitor Residential Care Facilities
and Skilled Nursing Facilities for the elderly and respond
to complaints. Volunteer Ombudsman will investigate
complaints and try to accomplish resolution of the problem.

Outreach - Provides information
and encouragement about existing services and benefits
to individuals in Fresno and Madera Counties.
