|
MSSP is a Medi-Cal funded program offered
locally by the Fresno-Madera Area Agency on Aging with
no charge for services. It is multidisciplinary case
management program coordinated by a Public Health Nurse
and a Social Worker. MSSP provides a comprehensive health
and psychosocial assessment. An individualized care
plan is developed in collaboration with the client,
family, physician and other service providers. The MSSP
assessment takes place in the client's home. Monthly
telephone contacts and quarterly home visits are conducted
to monitor service delivery and ensure client safety.
