In a community-based system of care, the focus of serving the whole person includes provision of supportive services including Information and Assistance, Outreach, Case Management, Adult Day Care, Legal Assistance, Transportation, and Telephone Reassurance.

With approximately 185,000 Congregate Meals and over 390,000 Home-Delivered Meals served annually throughout Fresno and Madera counties, nutrition remains a vital service of the Agency on Aging. 

The Family Caregiver Support Program provides individuals who care for a family member or friend with practical tools to alleviate stress and avoid burnout, networking opportunities to decrease isolation and share concerns, and stop-gap measures to stabilize the provision of care during times of extreme difficulty. An array of services are offered under the categories of information, access, support, respite care, and supplemental services.

The FMAAA provides funding for the Health Promotion Program (Title III D), Health Insurance Counseling and Advocacy Program (HICAP), Long-Term Care Ombudsman Program (Title III B, Title VII (b), and Federal Citation Penalties Account), and Elder Abuse Prevention, Education, and Training (Title VII).
