Care Management

The Fresno-Madera Area Agency on Aging’s care management team is trained to assess an individual’s physical, psychological, and social needs, financial resources, and the strengths and weaknesses of their informal support system. Their assigned care manager will then create a care plan that will maximize the participants functional independence and provide a dignified and safe home environment.

The Fresno-Madera Area Agency on Aging provides two care management programs.

Multipurpose Senior Services Program (MSSP)

This care management program, commonly called MSSP, is a multidisciplinary care management program coordinated by a Public Health Nurse and a Social worker. The purpose of MSSP is to promote client independence by coordinating home and community-based services and resources.

MSSP Eligibility
  • 65 years of age and older
  • Receiving Medi-Cal benefits
  • Frail health and at risk of nursing home placement
  • Reside in Fresno or Madera County
  • Have a need for Care Management
  • Be willing to participate in the program

Linkages Care Management Program

This care management program provides a client driven plan of care, coordination of services, advocacy, ongoing support and education for clients and their caregivers.

Linkages Eligibility
  • 60 years of age or older
  • Reside in Fresno or Madera County
  • Able to be maintained in the community through the use of case management services
  • Have a need for care management
  • Be willing to participate in the program
